This program gives each member and their spouse participating in the Plan 15 credits per month to use at selective gyms, classes or online.
Initial registrations must be done on a desktop computer using the below link.
Ignite Wellness Program (Expiring in 2025)
Subscribers & Spouses/Domestic Partners accumulate additional points by completing various wellness activities to use towards gift cards.
1 point = 1 dollar
Maximum 200 points to redeem for a gift card
The Requirements MUST be completed by December 31, 2025.
Annual Screening Form (Expiring 2025)
The Screening & Preventative Form is part of the Ignite Wellness Program. Download the form and have your provider fill out. Then upload at iaff587.medikeeper.com to earn 50 points.
